We're located just south of the corner of 22nd Avenue North and Martin Luther King Street on the West side of the street.
From I-275, exit on 22nd Avenue North and go East on 22nd Avenue North to Martin Luther King Street. Turn right—we're on the next corner on the right.
Licensing
Weller Chiropractic is comprised of individual practitioners who each maintain separate practices. When applicable, each practitioner is licensed by the State of Florida to engage in his or her particular health care field of practice, which carries with it the responsibility to comply with the licensing regulations of that particular field of practice.
Weller Chiropractic is a licensed massage establishment and complies with the regulations set forth by the State of Florida Department of Professional Regulations. Establishment license # MM 13320
Records and Fee Schedules
Weller Chiropractic practitioners maintain separate financial and patient records. Each practitioner collects payment for services in accordance with his or her individual fee schedules.
Liability and/or Malpractice Insurance
When applicable, each Weller Chiropractic practitioner is responsible for maintaining professional liability and/or malpractice insurance as dictated by the State of Florida Department of Professional Regulations.
